This Settlement offers to pay money to everyone who lived at Church Street South during some or all of the time period from December 19, 2013 until December 19, 2016. Some people may be eligible for other benefits.
The deadline to submit a Base Pay Claim Form was June 14, 2021 and has now passed.
Proof of Residency:
Claimants who were not listed on the official Church Street South rent rolls as authorized tenants also needed submit documentation establishing that they lived at Church Street South at some time during the Class Period, December 19, 2013 to December 19, 2016, in order to qualify under the Settlement. The deadline to submit this documentation was June 14, 2021, and has now passed. The documentation must have been received from, or submitted to, any federal, state, or local governmental entity during the Class Period. Examples of documents that would satisfy this requirement include, but are not limited to, tax forms, benefit applications or payments, driver’s licenses, motor vehicle registration, and school registration documents or correspondence.
Enhanced Payment Claim Form:
The official deadline to submit an Enhanced Payment Claim Form was June 1, 2021. However, you are still encouraged to submit such form if you have not already, if you believe that it applies to you or a family member. Enhanced Payment may be available if you or your family member suffered serious injuries or serious property damage caused by mold at Church Street South. The Enhanced Payment Claim Form is located in the Important Documents section of this website.
